This may include, but is not limited to, my name, contact information, identification number, business details (if applicable), and relevant information for social service eligibility.

I acknowledge and confirm that the City Government of Trece Martires and its offices are authorized to share the aforementioned information with legitimate authorities or institutions upon their request. This consent is in accordance with the provisions of the Data Privacy Act of 2012 (RA 10173).
