I hereby declare and give my consent to the City Government of Trece Martires for my participation in the Citizen Masterlisting, including but not limited to business profiling and other social services within the City Government of Trece Martires.

I further confirm that the City Government of Trece Martires and its offices involved in citizen masterlisting and other social services to collect, lawfully use, and disclose my personal information. This may include, but is not limited to, my name, contact information, identification number, business details (if applicable), and relevant information for social service eligibility.

I acknowledge and confirm that the City Government of Trece Martires and its offices is authorized to share the aforementioned information with legitimate authorities or institutions upon their request during the processing of my application for citizen masterlisting and verifying my identity for business profiling and other social services. This consent is in accordance with the provisions of the Data Privacy Act of 2012 (RA 10173), where “I certify that the information given is true and correct.”

I understand that, in the processing of my personal and sensitive personal information, the City Government of Trece Martires is committed to strict compliance with the Philippine Data Privacy Act and other related laws and issuances.

I understand that the City Government of Trece Martires and other involved offices for citizen masterlisting are authorized to process my personal and sensitive personal information without my consent pursuant to the relevant portions of Sections 4.12 and 13 of the Philippine Data Privacy Act.

I hereby give my consent freely and voluntarily, fully understanding the implications of providing such consent for citizen masterlisting, business profiling, and other social services within the City Government of Trece Martires.
